This Acknowledgement of Service can be used as confirmation that documents were served on a person.
The acknowledgement is prepared by the party serving the documents (or their lawyer) and must include a list of all the documents being served.
This form is designed to generate the prescribed form for the:
➲ Federal Circuit and Family Court of Australia; or
➲ Family Court of Western Australia; but it can also be used to
➲ Generate a draft Acknowledgement of Service for use in other Courts so long as you contact our legal team so that we can ensure the correct prescribed form (if applicable) for the relevant Court is used, and it is correctly filed at the relevant Court registry (if required).

✅ The person serving documents by hand must give a copy of the documents and the Acknowledgment of Service to the person to be served.
✅ They must ask the person to be served to add in the date they were served and sign the Acknowledgment of Service at Part C.
✅ The acknowledgement is to be signed by the party that is served the documents (or their lawyer) in confirmation that they received the documents listed.
✅ If the person to be served does not take the documents, the person serving may put it down in the presence of the person to be served and tell the person what it is.
✅ The server must complete an Affidavit of Service and attach the Acknowledgment of Service and return the documents to the applicant.
Note: If your matter is in the Family Court of Western Australia you will need to use this alternate Affidavit of Service.
✅ Once the Acknowledgment of Service is returned to the applicant it must be filed.
✅ An Acknowledgment of Service for either the Federal Circuit and Family Court of Australia or the Family Court of Western Australia can be filed on the Commonwealth Courts Portal.
