This document follows the format of prescribed Form 1 for Enduring Power of Guardianship contained in Schedule 1 of the Guardianship and Administration Act 1995 (TAS) and the form approved by the Guardianship and Administration Board of Tasmania.
Use this form to appoint Enduring Guardians and an Alternative Enduring Guardians to make decisions for personal matters.
The document must be lodged for registration with the Guardianship and Administration Board of Tasmania to be valid.
Use an Enduring Power of Attorney to appoint Attorneys for financial matters.
1️⃣ Appoint up to 3 Enduring Guardians and 1 Alternative Enduring Guardian to make decisions for personal matters;
2️⃣ Authorise all functions of an Enduring Guardian as permitted by Guardianship and Administration Act 1995 (TAS); and
3️⃣ Impose any conditions upon the appointment of Enduring Guardian(s).
